    By submitting and signing this form, I acknowledge and understand:

    • Submit this form each semester you wish to use VA Education Benefits.
    • I must be registered for courses before my enrollment can be certified to the Department of Veterans Affairs (VA).
    • Only courses that apply toward my declared program of study will be certified for VA benefits.
    • I am responsible for notifying the School Certifying Official of any enrollment changes, including adds, drops, or withdrawals.
    • I understand that changes to my enrollment may result in a debt with the VA, and I am responsible for any overpayment.
    • I understand that course format and term length (including non-standard terms) may impact my rate of pursuit and VA benefit payments.
    • I certify that all information provided is accurate and complete, and I understand that incorrect information may delay or impact my benefits.
